Thanks to Crunchy B. for getting in touch with info and photos of this booth, "a Model 21 located in the Block 16 food court on the second floor of the Cosmopolitan of Las Vegas, sitting in between the restaurants Lardo's and Naughty Patty's. A sign on the front says this one is owned by Victoria and Stephen, who also run a few others in town. This booth also boasts 24-hour access."
